New Zealand actor Sam Neill, beloved for his memorable performances in Jurassic Park and The Piano, has died at the age of 78. His family confirmed that he died suddenly in Sydney, describing his passing as “sudden and unexpected” in a statement shared on his social media accounts.
Neill had publicly revealed in 2023 that he was living with angioimmunoblastic T-cell lymphoma (AITL), a rare and aggressive form of non-Hodgkin lymphoma, after being diagnosed in 2022. Although his family later confirmed that his cancer had gone into remission before his death, his battle drew global attention to a little-known blood cancer that primarily affects older adults.
According to a study published in the Journal of Family Medicine and Primary Care, non-Hodgkin lymphoma accounts for nearly 5 per cent of head and neck cancers, highlighting the importance of early recognition and timely diagnosis.

What is angioimmunoblastic T-cell lymphoma (AITL)?
Angioimmunoblastic T-cell lymphoma (AITL) is a rare subtype of peripheral T-cell lymphoma, which belongs to the broader group of non-Hodgkin lymphomas. It develops in T lymphocytes (T-cells) – white blood cells that play a crucial role in protecting the body against infections.
Unlike many cancers that arise in organs, AITL affects the immune system itself. This weakens the body’s ability to fight infections and can trigger abnormal immune responses. The disease is most commonly diagnosed in people over the age of 60 and accounts for only a small percentage of all non-Hodgkin lymphoma cases.
Symptoms often mistaken for common illnesses
One of the biggest challenges with AITL is that its symptoms can resemble viral infections or autoimmune disorders, delaying diagnosis. Common symptoms include:
- Swollen lymph nodes in the neck, armpits, or groin
- Persistent fever
- Night sweats
- Unexplained weight loss
- Severe fatigue
- Skin rashes or persistent itching
- Frequent infections
- Enlarged liver or spleen
Doctors advise seeking medical evaluation if these symptoms persist for several weeks without an obvious cause.
How is it diagnosed?
Diagnosing AITL usually requires a combination of clinical examination and specialised testing. Doctors may recommend physical examination, blood tests, lymph node biopsy (the gold standard), CT scan or PET-CT scan, bone marrow biopsy, if necessary, and advanced laboratory tests to identify the lymphoma subtype.
A lymph node biopsy remains the most definitive way to confirm the diagnosis.
Sam Neill’s treatment journey
After his diagnosis, Sam Neill initially underwent chemotherapy. However, when the treatment stopped working, he received CAR-T cell therapy, an advanced form of immunotherapy that uses a patient’s own immune cells to attack cancer.
The treatment successfully brought his lymphoma into remission, and his family later confirmed that recent scans had shown no detectable cancer before his passing.
What is CAR-T cell therapy?
CAR-T (Chimeric Antigen Receptor T-cell) therapy is one of the most significant breakthroughs in modern cancer treatment. According to research published in Molecular Medicine Reports, CAR-T therapy genetically modifies a patient’s own T-cells to recognise and destroy cancer cells more effectively.
The process involves collecting T-cells from the patient’s blood, engineering them in a laboratory to target cancer cells, growing millions of these modified cells, and infusing them back into the bloodstream. Often described as a “living drug,” CAR-T therapy has transformed treatment for several blood cancers and is now being investigated for solid tumours and autoimmune diseases.
What are the treatment options?
Treatment depends on the patient’s age, disease stage, and overall health. Options may include chemotherapy, immunotherapy, targeted therapy, stem cell transplantation, and CAR-T cell therapy for relapsed or treatment-resistant disease.
Many patients receive a combination of treatments to improve outcomes.
Can Angioimmunoblastic T-Cell Lymphoma be prevented?
There is currently no proven way to prevent AITL. Researchers are still studying its exact causes, although advancing age, immune system dysfunction, and genetic abnormalities are believed to increase the risk. While prevention isn’t possible, early diagnosis can significantly improve treatment options and outcomes. Anyone experiencing persistent swollen lymph nodes, unexplained fever, night sweats, weight loss, or ongoing fatigue should consult a healthcare professional rather than assuming the symptoms are due to a routine infection.
Sam’s courageous journey not only raised awareness about angioimmunoblastic T-cell lymphoma but also highlighted the remarkable advances being made in blood cancer treatment, offering renewed hope to patients facing this rare and aggressive disease.
